Patient Responsibilities
It is your responsibility to provide accurate personal information to Letourneau’s Pharmacy in order to ensure proper care, treatment, and service, billing, delivery of medication, supplies, or equipment, and/or contact in the event of an emergency.
It is your responsibility to provide proper and accurate insurance information.
It is your responsibility to provide a complete and accurate medical history. This history should include all prescribed and over-the-counter medications that you are taking.
It is your responsibility to inform us about all treatments and interventions that you are involved in.
It is your responsibility to follow the suggestions and advice prescribed in a course of treatment by your health care providers.
If your refusal of treatment prevents us from providing care according to ethical and professional standards, we may need to end our relationship with you after giving you reasonable notice.
It is your responsibility to accept the consequential outcomes if you do not follow the instructions you receive at Letourneau’s Pharmacy.
It is your responsibility to be considerate of the rights of other patients and Letourneau’s Pharmacy personnel and property.
It is your responsibility to provide information about unexpected difficulties or changes you experience involving your health care.
It is your responsibility to make it known whether you clearly understand your plan of care and the things you are asked to do.
It is your responsibility to provide us with correct information about your sources of payments and ability to pay your bill.
If you have any concerns or complaints about your treatment, care, or service you can notify any staff member and they will help you in properly filing your concern or complaint.
